APPLICATION FORMS
We have TWO application forms.

The first is an "on-line" AUTOMATED application. Fill out the form by tabbing from field to field, print a copy for yourself, and hit the "submit" button to send it directly to CCDAL.

Automated Application


The second is in MS Word. You can either print the form from your browser, or download and edit the file via the link below. The following steps should help you complete and submit the form:

  • Click on the MS Word version of the form below.
  • Click on the "File" menu and choose "Save as..."
  • Remember where you saved the file for later e-mail.
  • Set your keyboard to "overwrite" to fill in the blanks.
  • Complete the form on your computer.
  • Save the final version of the form back to your computer.
  • Print or email the saved version to CCDAL for approval.

Application in MS Word

If you simply print out the form, you can fill it in by pen and mail it to:

PLEASE NOTE: An application sent by mail can take two weeks to get reproduced and re-mailed to the Application Review Committee, so the preferred method of receipt is by the automated version or email.
